Role Overview
The post holder will work as a mental health practitioner within the Crisis Resolution Home Treatment Team based in HUB – Fixed term for Maternity Cover. The overall objective is to provide a very high standard of care to patients under the care of the Crisis Resolution Home Treatment Team as an alternative to an acute mental health admission.
ResponsibilitiesThe post holder will provide high quality and comprehensive assessments, treatment and brief, evidence-based follow up service to patients referred from the community mental health teams; psych liaison service or inpatient wards.
The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. The post holder will ensure that each individual plan is developed in consultation/agreement with the service user and informed by the wider multidisciplinary team.
The post holder will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user’s recovery.
The post holder will be confident in taking positive risk in accordance with the Trust Policy around Risk Assessment and management.
- The post holder will be a member of the Crisis Resolution Home Treatment Team based in the main HUB.
- The post holder will provide assessments to patients 18+ yrs, following appropriate induction and training.
- Maintain and develop relationships with professionals external to the area of service, service users and their significant others (where appropriate) within area of service/specialism.
- Be responsible for the assessment, planning, implementing and evaluation of specialist care, including evaluating the work of junior staff, and to be responsible for development and delivery of intensive crisis care plan, working collaboratively with multi‑agencies to ensure service users’ needs are met.
- Formulate and design physical interventions as part of the service user’s care plan, ensuring that clinical policies and procedures are followed correctly and risks inherent in any procedures are minimised.

FOR SECONDMENT ROLES ONLY – If the secondment role becomes permanent, the successful candidate may be offered the permanent position.
Must Haves- Must hold a relevant qualification either registered mental health nurse or social worker
- Sound understanding of the acute care pathway
- Assessment and case formulation, evidence of skills utilised to manage risk
