We are looking for a motivated and experienced
Clinical Pharmacist to join our team. You will be part of a team of five Clinical Pharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to the practice and its patients through primary care. You would be based at Farnham Practice but would be expected totravel to our other locations and our PCN SPINE partner, Kumar Medical Centre,within Slough, to be involved in the wider runningof the practice, such as participating in clinical meetings and contributing to the development of new services.
You will be part of the Clinical Pharmacist team ensuring prescribingmedication is safe, effective, and is optimised for and with our patients. The job description and Person Specificationfor the role are attached.

Main duties of the jobTowork closely with SPINE PCN GPs and act as point of contact in the practice forall medicines related queries for health care professionals and patients.
To manage the repeat prescribing reauthorisation process and undertakemedication reviews of patients, some of which may need to be undertaken in the patients home.
To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF and Locally Commissioned Services.
Manage practice formularies to improve the quality, safety, and cost effectiveness of prescribing.
Where appropriate, review pathology results for patients on known medicines.
To help plan, develop and support the introduction of new working processeswithin the practice to optimise the quality and safety of prescribing and undertake medication audits.

Job responsibilitiesWorking closely with SPINE PCN GPs
- Ips to resolveday-to-day issues relatedto medication.
To act as point of contact in the practice for all medicines related queries for health care professionals and patients.
To manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
To undertake medication reviews of patients including those with multiple long term conditions and polypharmacy to improve patient outcomes and maximisecost-effective prescribing. Some reviews may need to be undertaken in the patientshome.
Ensure that the practice repeatprescribing policy is regularly reviewedand that it is beingadhered to with the aim of reducing waste and improving safety.
Contribute to reductions in medicine-related hospitaladmissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues.
To support patientsrecently discharged from the hospital by ensuring they understand their medication, any changes that have happened recentlyand have a timely supply.
To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
To follow up issues highlighted by Medicines Use Reviews carried out in local Community Pharmacies.
