Advanced Nurse Practitioner

We are looking to recruit an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly and supportive GP practice team. This is an exciting opportunity for a highly skilled clinician to work autonomously within primary care, delivering high-quality, patient‑centred care.

The successful candidate will assess, diagnose, and manage a wide range of acute and chronic conditions, working as part of a multidisciplinary team to support the delivery of effective and accessible services to our patient population.

• Provide advanced clinical assessment, diagnosis, and treatment for patients presenting with undifferentiated conditions
• Independently prescribe medication in line with current guidelines and best practice
• Manage same-day appointments, minor illness, and urgent presentations
• Undertake clinical management of patients with long‑term conditions where appropriate
• Work collaboratively with GPs, practice nurses, pharmacists, and wider PCN teams
• Contribute to clinical audits, quality improvement, and QOF delivery
• Support triage systems and improve patient access to care
• Maintain accurate and contemporaneous patient records using clinical systems (EMIS)
• Participate in practice meetings and contribute to service development

Key Responsibilities

The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice, providing high‑quality clinical care to patients registered at the practice. The post holder will be responsible for assessment, diagnosis, treatment, and evaluation of care for patients presenting with a wide range of undifferentiated and undiagnosed conditions.

The ANP will work collaboratively with the multidisciplinary team to support safe, effective, and responsive primary‑care services.

• Provide expert clinical assessment, diagnosis, and treatment for patients with acute and chronic conditions
• Manage patients presenting with undifferentiated and undiagnosed illnesses
• Prescribe medications independently in accordance with prescribing regulations and practice protocols
• Undertake clinical decision‑making and risk management in complex situations
• Deliver evidence‑based care in line with national and local guidelines
• Refer patients appropriately to secondary care or other services

• Support same‑day access clinics and triage systems
• Contribute to improving patient access and flow within the practice
• Participate in the management of long‑term conditions where appropriate
• Promote health education and self‑management to patients

• Work collaboratively with GPs, nurses, pharmacists, and administrative staff
• Participate in multidisciplinary team meetings
• Provide clinical advice and support to colleagues where appropriate
• Contribute to the development of integrated working within the Primary Care Network (PCN)

• Registered Nurse with current Nursing and Midwifery Council (NMC) registration
• Independent Non‑Medical Prescribing qualification
• Post‑registration qualification in clinical assessment and examination skills (or equivalent experience)
• Evidence of ongoing Continuing Professional Development (CPD)

• Experience of working in a Primary Care Setting
• Working knowledge of using EMIS
