What you’ll be doing :

As Reablement Social Work Practice Lead, you’ll play a key role in supporting adults and carers to achieve positive outcomes, maximise independence and access the right support at the right time. Working within a multi-disciplinary Reablement and Independence Service, you’ll provide professional leadership, guidance and supervision to Reablement Social Workers, while also holding a small caseload of more complex work.

You’ll undertake strengths-based assessments, care and support planning, reviews, risk assessments, Mental Capacity and Best Interests assessments, ensuring practice is person‑centred, legally compliant and focused on promoting independence. You’ll work closely with therapy colleagues, locality teams, hospital social work teams and external partners to support joined‑up working and smooth transitions where longer‑term care needs are identified.

You’ll support evidence‑based approaches, contribute to audits, provide advice on complex cases, support safeguarding activity where required, and ensure accurate records are maintained so residents and carers receive high‑quality, timely and effective support.
