As a Physical Therapist on our skilled nursing facility team, you’ll empower healing and restore independence, delivering a meaningful impact in the lives of residents every step of the way.
Position Summary
The primary function of this role is to return patients to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a skilled nursing facility (SNF) setting. Instruct patients in maintaining their achieved level of function after discharge through education, independent exercise programming, and appropriate activity modification.
Responsibilities
- Perform comprehensive physical therapy evaluations and design individualized treatment plans.
- Deliver hands‑on therapy care and monitor progress toward rehabilitation goals.
- Educate patients, families, and care teams on functional maintenance and safety post‑discharge.
- Document assessments, interventions, and outcomes in the electronic health record.
- Collaborate with physicians, nurses, and interdisciplinary staff to optimize patient outcomes.
Education / Qualifications
- Bachelor’s degree or higher in Physical Therapy from an accredited college or university.
- Active, unencumbered license in the state of employment.
- Basic computer skills and proficiency with service‑delivery and electronic health record software.
Shift & Wages
- Full‑time and part‑time opportunities available; shift varies.
- Sign‑on bonus: $10,000.
