Registered Dietitian

Evergreen Post Acute is hiring a full-time Registered Dietitian to join the clinical team at our 151-bed skilled nursing and rehab community in Smyrna.

• One building, 151 beds
• Skilled Nursing & Rehab Facility
• Salary: $70,000–$85,000, based on experience
• Sign On Bonus

• Complete admission, quarterly and significant-change nutrition assessments for post-acute and long-term residents
• Develop individualized care plans.
• Participate in interdisciplinary care conferences and weekly risk meetings, where your recommendations shape outcomes
• Contribute nutrition sections for MDS and support accurate PDPM capture
• Document in Point Click Care

• Active RD/RDN credential in good standing
• Delaware LDN license, or eligibility to obtain one
• Bachelor's degree or higher from an ACEND-accredited program
• Preferred, not required: long-term care experience and familiarity with MDS, PDPM and NFPE. If you're coming from acute care or outpatient work, we'll train you on the LTC side.
