Dialysis HHD PD Registered Nurse RN at Fresenius Medical Care

As a Home Therapies Registered Nurse, you will manage patient care in an independent home therapy program by providing education and support to patients and caretakers who have chosen to dialyze in the comfort of their home. This role is primarily clinic based, you will conduct initial visits to patients' homes to assess readiness and will provide regular training and follow-up visits within the clinic environment.

Principal Duties and Responsibilities
  • Participate in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
  • Identify and evaluate candidates for the Home Therapies Program including assessment of patients’ home environment as needed.
  • Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and caretaker readiness and ability to perform dialysis treatments in the home.
  • Admit new patients to the Home Therapies Program using the appropriate processes.
  • Coordinate patient care (assessment, planning, intervention, and evaluation) for an assigned group of hemodialysis patients.
  • Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols.
  • Analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed.
  • Provide initial and ongoing education to patient and family on home dialysis treatment and operation and maintenance of all home therapies equipment.
  • Ensure patient awareness related to transplant and treatment modality options.
  • Administer medications as prescribed or in accordance with approved algorithm(s), documenting appropriate medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Complete routine patient evaluations within the clinic setting.
  • Ensure correct laboratory collection, processing, and shipping procedures are performed.
  • Collaborate with the Interdisciplinary Team on the patient care plan.
  • Make referrals to Social Worker and Registered Dietitian as appropriate.
  • Complete CAP requirements to maintain or advance within the program.
Education
  • Graduated from an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Meet practice requirements in the state of employment.
Experience and Required Skills
  • Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
  • Experience as a Registered Nurse (preferred but not required).
  • Chronic/home hemodialysis experience (preferred but not required).
  • Successfully pass the Ishihara Color Blind Test.
Physical Demands and Working Conditions
  • Job involves heavy lifting, moving patients, and assisting with ambulation with aid as needed.
  • Requires frequent, prolonged standing and ability to bend over.
  • Possibly move machines and equipment up to 200 lbs and lift 30 lbs solutions up to 5 ft.
  • Work environment is a health care facility with controlled temperature and moderate noise.
  • May be exposed to infectious and contagious materials.
  • Work includes desk and computer work, patient interaction, and coordination with staff and physicians.
  • May require travel to patient homes, training sites, or other facilities.
  • May need to provide essential functions at other locations with same physical demands.
