Description
The Twelve Stone Infusion Centers LLC manages infusion needs and maintains continuum of care from intake to completion. Our primary focus is safe and effective administration of biologic medications such as Remicade and Tysabri, while offering a full range of infusion services including antibiotics, hydration, vitamins, iron, steroids, sterile dressing changes, and lab draws.

PositionWe are currently hiring a Nurse Practitioner for our infusion center. This NP will provide comprehensive care in an outpatient setting, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients and families with acute and chronic health needs.
Responsibilities- Provide patient and family-centered IV infusion care and facilitate consistent, coordinated care among all team members.
- Incorporate patients and families into care planning and treatment across settings and serve as an advocate for navigating the health care system.
- Communicate and collaborate effectively with physicians, health professionals, agencies, patients, and families; promote mutual respect and trust within a team model.
- Review, update, and recommend relevant medical, clinical, and administrative policies and procedures to assure high-quality patient care.
- Actively participate in quality initiatives; maintain accurate and timely records and reports; participate in patient service and program reviews.
- Other duties as assigned.
Master’s degree in nursing from an accredited nursing school. Active and unrestricted Advanced Practice Registered Nurse and Registered Nurse license in the state where you practice, as well as current active board certification with ANCC or AANP (or equivalent NCCPA license). Current CPR certification (BLS minimum, including hands‑on/in‑person assessment). Minimum 2 years of NP or PA experience in a clinical setting (or 3+ years as a Registered Nurse in oncology, ER, ICU, or infusion center).
Infusion experience required. Demonstrated high degree of clinical expertise with acute and chronic illnesses.
