Position Summary
Smyrna Care Center is a full-service nursing facility specializing in complex care. We are committed to providing high-quality clinical and rehabilitative services in a compassionate, supportive environment designed to meet the evolving needs of our patients and residents.
LPNs are responsible for providing compassionate, high-quality care to residents while ensuring compliance with all regulatory requirements. This role involves direct clinical care, documentation, communication with interdisciplinary teams, and supervision of nursing staff

Apply today and hear back within 48 hours - Perform daily rounds and assessments on assigned residents.
- Monitor and ensure delivery of high-quality resident care.
- Administer medications and maintain accurate documentation.
- Communicate changes in condition to residents, families, and healthcare providers.
- Assist in developing and updating resident care plans.
- Support and train nursing staff, including new and temporary employees.
- Ensure safety and compliance with OSHA, infection control, and emergency protocols.
- Participate in quality assurance and state survey processes.
- Direct supervision of nursing staff and indirect supervision of all staff involved in resident care.
- Involved in hiring, training, performance management, and staff scheduling.
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to supervise, lead, and mentor nursing staff
- Physical ability to lift up to 75 lbs and perform other duties as required
- Commitment to resident dignity and quality of life
- Graduate of an accredited nursing program.
- 1+ year of nursing experience preferred

- Must hold a valid
LPN License
in the state of employment.
