Position Summary
Advocate for clients, obtain entitlements, coordinate transportation between ChangePoint and other service providers. The BHT/Case Manager is expected to cooperate as a team member in the provision of a therapeutic environment that encourages each customer's emotional growth. This position will be expected to maintain an acceptable level of productivity as measured by billable hours. This position is expected to meet all standards for clinical documentation.
PrincipalDuties / Responsibilities
- Adheres to personnel policies and complies with agency requirements for ethical and professional behavior.
- Provides services to clients as a case manager.
- Is responsible for all aspects of the clinical records, provides crisis services and assists with service coordination for the client.
- Engages and includes the client in the development and implementation of the treatment plan.
- Protects client confidentiality and personal rights.
- Keeps clear, concise and accurate records of clinical activity according to Policy and Procedures.
- Assists clients in obtaining entitlements and benefits pertaining to housing, income, health and other supportive services, as needed.
- Provides recreation, leisure, and pro-social skills for clients both individually and in groups.
- Provides transportation as needed.
- Works with other agencies to provide wrap services including going in the home and/or school to prevent out of home placement.
- Provides assertive outreach to clients who resist involvement or have difficulty accessing services.
- Establishes and maintains a working relationship with personnel and referral resources.
- Performs vocational screenings and evaluations.
- Act as a liaison between client, Department of Vocational Rehabilitation, and employer.
- Provide job development and placement services.
- Provide community training site development.
- Coordinates and accompanies clients to PCP/Doctor appointments.
- Provides health promotion groups including but not limited to hygiene, exercise, and nutrition.
- Must meet state standards for continued education.
- If qualified, provides rotating on-call coverage and update tx plans.
- Promotes quality and safety to help maintain a safe and pleasant work environment, follows safety regulations, and actively contributes towards a safe workplace.
- Must participate in quality management and quality improvement activities.
- Performs other duties as assigned by their supervisor or designee.
Required: High School Diploma plus five (5) years Behavioral Health Experience for BHT case management level, Current Arizona Driver's License, Compliance with a Drug Free workplace, Must be at least 21 years old, Knowledge and ability to define the needs of the client served.
Skills / AbilitiesRequired: Fluent in English, both verbally and in writing;
Literate;
Able to effectively employ analytical and problem‑solving skills;
Able to prioritize and handle multiple tasks with customers;
Ability to accurately complete difficult paperwork in a timely manner producing professional documents, charting, and record keeping using TJC, NARBHA and DHS standards.
Preferred: Experience and training in crisis intervention;
Familiar with psychotropic drugs and their side effects.
- Able to sit for extended periods of time.
- Able to speak clearly in ordinary conversation and phone communications.
- Able to hear ordinary conversation and phone communications.
- Able to work with customers who are emotionally distressed, in crisis.
- Able to work with individuals who have been abused or perpetrated abuse.
- Able to work with customers who abuse substances.
- Able to drive a vehicle.
- Able to operate general office equipment.
- Air conditioned and/or heated office setting.
- Will accommodate as appropriate and reasonable.
