JOB SUMMARY:

Supports the implementation ofthe Hooper Bay Wellness Project an average of 10 hours/week. Assists with planning and hosting activities in the community that have been planned with the Hooper Bay Wellness Coordinator.

ESSENTIAL FUNCTIONS,

DUTIES AND RESPONSIBILITIES:

1. Assists with the design andimplementationand evaluation of the program services with input received from project staff,stakeholders and partners.

2. Conducts outreach activities in the community and helps to ensure logistics of events and activities are planned and implemented.

3. Works with the HB Wellness Coordinator to the provide logistical support and coordination of program events and meetings. Prepares and distributessupportivematerials.

4. Works with community leaders, projectstaffand partners to publicize and promote project activities and accomplishments.

5. Submits regular progress reports and responds to requests for information on project activities. Works with and supervisor to implement a program evaluation in the community.

6. Participates in regular supervisory meetings with the Hooper Bay Wellness Coordinator and all required training.

7. Networks with other RurAL CAP staff, service providers, preventionorganizationsand agency partners to promote the mission and goals of RurAL CAP.

8. Performs other duties as assigned.
