Registered Nurse, Rehabilitation

Join Lifepoint Health® as a Registered Nurse, Rehabilitation in the Rehabilitation Unit. This PRN position reports to the Post Acute Director and provides direct patient care using the nursing process in accordance with applicable scope and standards of practice.

Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

Reports to:

The Post Acute Director.

• Associate's degree required;
  Bachelor's degree preferred;
  Graduate of a Credited Nursing Program.
• Current RN license to practice in Kentucky or an active multistate license.

• American Heart Association BCLS.
• New employees with BLS certification from another organization (e.g., American Red Cross) have 30 days to complete the American Heart Association version.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, and other medical care treatments.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
• Assists patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Collaborates across disciplines to coordinate patient care, including transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Advocates for the physical and emotional well-being of the patient.
• Assesses, plans, implements, and evaluates care for a given population based upon age specific and cultural needs in a collaborative multidisciplinary approach.
• Ensures quality of care by measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital philosophies and standards of care set by governing agency regulations.
• Coordinates delivery of patient care for the Rehab Unit, facilitates problem solving, and communicates necessary issues to nursing administration and Program Manager.
