Social Worker needed for our Adult Social Care - Learning Disabilities team
in Somerset, paying up to £38 ph ! Hybrid Working!
  • Location: Somerset
  • Rate: £38 ph
  • Working Arrangements: Flexible / Hybrid - 2 days on site. expected travel within Somerset.
About the Role:

We are currently seeking an experienced Adult Social Worker to join a multidisciplinary team in Somerset. This is an excellent opportunity for a qualified Social Worker with strong Learning Disability practice experience and a solid background in Adult Social Care.

The successful candidate will be responsible for undertaking assessments, reviews and reassessments, identifying eligible care and support needs, and making evidence-based decisions that support positive outcomes for adults.

Key Responsibilities:
  • Complete Care Act assessments, reviews and reassessments for adults with care and support needs.
  • Work collaboratively within a multidisciplinary team
    , contributing professional Social Work expertise.
  • Identify and evidence eligible care and support needs in accordance with relevant legislation and guidance.
  • Undertake robust, evidence-based assessments and demonstrate strong analytical skills.
  • Exercise professional judgement and make defensible, proportionate decisions
    .
  • Work effectively with Social Care, Health Services, voluntary organisations and other partner agencies.
  • Consider and contribute to Continuing Healthcare (CHC) processes, including Checklists and Decision Support Tool (DST) assessments.
  • Understand and navigate joint funding arrangements and the boundaries between health and social care responsibilities.
  • Challenge funding decisions appropriately where evidence and professional rationale support doing so.
  • Maintain accurate records and effectively use IT systems for recording, retrieving and managing information.
  • Manage a varied workload, working autonomously and at pace while maintaining professional standards.
  • Travel across Somerset as required and attend the office at least two days per week
    .
