Personal Care Provider

Community Partners NH is a non‑profit organization that provides critical supports and services for individuals and families living with developmental disabilities and/or behavioral health challenges.

An in‑home personal care provider works 1:1 with children and adolescents with intellectual disabilities. The provider supports the individual on goals established as part of their In‑Home Supports plan. Goals may focus on increasing independence with activities of daily living, increasing social and communication skills, and gaining life skills.

• Work 1:1 with the individual to meet goals in the In‑Home Supports plan.
• Support independence with activities of daily living.
• Facilitate social and communication skill development.
• Achieve life‑skills milestones.
• Make community connections on behalf of the child.
• Maintain accurate and timely documentation.

• Experience working with individuals with disabilities, especially autism.
• Energetic, observant, and an excellent communicator.
• Comfortable identifying support needs and tailoring interventions.
• Valid driver’s license, clean driving record, proof of insurance, and able to provide transportation.
• Pass a background check, including criminal and driving records.
• Background of work as an LNA, CNA, DSP, Direct Support Professional, or related roles is considered an asset.
