• Conduct client assessments to identify needs and develop personalized case plans.
• Provide ongoing support and guidance to clients through regular check‑ins.
• Coordinate referrals to community resources and services.
• Document client progress and maintain accurate case records.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
• Advocate for clients' rights and access to necessary services.
• Participate in training and professional development opportunities.
• Assist in crisis intervention and conflict resolution when necessary.

• Bachelor’s degree in Social Work, Psychology, or related field preferred.
• Minimum of 1‑2 years of experience in case management or social services.
• Strong communication and interpersonal skills to build rapport with clients.
• Ability to work independently and as part of a team.
• Knowledge of community resources and social service programs.
• Compassionate demeanor with a commitment to client advocacy.
• Proficient in documentation and case management software.
• Valid driver’s license and reliable transportation required.
