Overview
This position is staffed within the Tribal Health Maintenance Program. The employee works under the supervision of the Director to provide services to tribal members to promote good health and reduce the risk factors in the area of chronic disease.
Responsibilities- Assists in developing and implementing public education materials and workshop sessions in the area of health promotion and chronic disease prevention.
- Conducts and/or coordinates education sessions one-on-one and in group settings to those with chronic diseases and their families, in areas such as nutrition, physical activity, medications, foot care, etc.
- Makes home visits to educate, monitor various conditions, take vital signs, deliver medication, explain their directions and provide assistance as needed.
- Refers patients to outside resources, services or higher levels of care.
- Assists families by providing information, education and supplies as needed.
- Maintains information on patient care, daily activities and program status.
- Prepares and submits monthly activity reports to Director.
- Attends staff meetings, training workshops, seminars as needed and required by Director and/or Administration.
- Records patient condition, treatment and/or assistance given in chart within one (1) day of providing services.
- Assists with other program activities as necessary and performs other duties as assigned by the Director.
- Transports members to non-emergency medical appointments.
- Confers with primary care providers as necessary for patient advocacy, scheduling and education.
- Attends weekly Case Management Meetings, as assigned.
- Assists with grant programs implementation and development as assigned.
- Will act as an interpreter/translator when needed.
- Administering Naloxone to individuals whom they believe, in good faith, are suffering from opioid overdose
Skills and Abilities
- Certified Nursing Assistant (CNA or Community Health Worker preferred.)
- Must be able to take vital signs and have basic knowledge of nutrition and physical fitness activities.
- Must be able to establish and maintain good interpersonal relationships with tribal members, THMP staff, IHS service unit staff, and tribal departments and outside agencies.
- Must have good speaking and writing skills.
- Ability to speak the Cocopah language preferred.
- Ability to drive a car/van.
- Must be familiar with local/community health resources.
- Must complete Indian Health Services Community Health Representative Basic Training Model within first 120 days of hire.
- Arizona/California Driver's License.
- Cardio-Pulmonary Resuscitation, First Aid & AED Certification.
- Testing may be required.
- Associates degree in related field or trade school, professional or technical certification, specialized training
- 1-year equivalent to at least TS-2, 6-months specialized work experience, 2-years work experience in related field or 2-years general or supervisory work experience
