RN Field Clinical Care Coordinator - Dedham, Needham, West Roxbury, MA

The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical and behavioral needs, focusing on care coordination to promote appropriate service utilization and improved quality of care.

• Pay: $29 - $52/hour
• Location: Dedham, MA
• Schedule: Full Time, Day shift
• Travel: Local travel up to 75% with mileage reimbursement

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic health conditions and member safety.
• Partner and collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Support proactive discharge planning and manage/coordinate care transitions following ER visits or admissions.
• Advocate for members and families to ensure their needs and choices are represented by the healthcare team.

• High School Diploma/GED required

• 2+ years of clinical experience required

• Current and unrestricted independent licensure as a Registered Nurse in MA required
