Somerville Homeless Coalition (SHC) is a human services agency helping families and individuals in all stages of homelessness and housing insecurity. Services include emergency shelter, housing search case management, daytime drop‑in, street outreach, supportive housing, homeless/eviction prevention and connections to supplemental services. SHC also operates Project SOUP, which is a food service program that includes a pantry marketplace, community meals and delivery of groceries to the homebound.

Provide community and home‑based case management and direct support to foster housing stability and the overall well‑being of clients residing in SHC’s housing programs. Intensively work with individuals and families, that are disabled and formerly homeless, on a regular basis to facilitate linkages to a range of services and resources. Provide crucial support and coaching around psychiatric symptom management, substance use and, behavioral health management, trauma and physical ailments.
Ensure all clients are compliant with program rules, individual service plans and lease requirements.
- Conduct comprehensive applicant interview and assessment process
- Meet with individuals and families in their homes and in the community on a regular basis—multiple times a week
- Provide crisis intervention, informal counseling support and advocacy services to promote health and safety
- Provide close support around complex dual diagnosis related issues and behaviors
- Assist with budgeting and credit repair needs; benefit attainment; access to healthcare; addressing court related issues; and medication monitoring; parenting education; connections to education, employment, and volunteer opportunities; referrals to mainstream services; and activities related to family, social and community reintegration
- Support clients in their moving on plans by applying for all affordable housing opportunities, monitoring application statuses and supporting them in increasing their household income
- Schedule and accompany clients to meetings and service‑related appointments
- Teach essential living skills, social skills, and healthy coping skills
- Develop, monitor, and implement individual service plans (ISP)
- Ensure housing lease compliance and maintenance of health and safety standards
- Organize and maintain accurate and up‑to‑date paperwork, including client files and program reports
- Enter comprehensive individual meeting notes and assessments into online database systems (HMIS)
- Organize curriculum for and facilitate peer support group(s)
- Serve as liaison to landlords, property management companies and other housing agencies
- Assist clients in maintaining apartments to meet housing quality standards by addressing sanitary and maintenance issues
- Assist with client housing relocation on an as needed basis
- Serve as intermediary and coordinator within each client’s network of service providers
- Cultivate positive working relationships with human service and government agencies
- Attend individual supervision, staff meetings, and community‑based meetings
- Perform supervised urine drug screening for clients on a consistent basis
- Provide limited after hours on‑call phone availability
- Demonstrate strong and clear communication both written and verbal
- Abide by handbook rules, policies and code of conduct
- Perform other related duties as assigned

- Car and valid driver’s license required
- Minimum one year relevant experience with homeless, mental health, and substance use services and resources
- Proficient computer and technology skills including, Microsoft Word and Excel and other web‑based applications
