Physical Therapist (Healthcare)
Physical Therapist (PT) - Sonoma Post AcuteLocation: Sonoma, CA
Pay: up to $70 per hour
Schedule: Full Time, Part Time, or Per Diem
Sonoma Post Acute is hiring Physical Therapists to join our therapy team. We are looking for caring and motivated professionals who want to make a difference in the lives of our residents.

Responsibilities
- Provide physical therapy services to residents in a skilled nursing setting
- Create and follow individualized treatment plans
- Work with nurses, physicians, and the rehab team to improve outcomes
- Document progress and ensure compliance with regulations
- Educate residents and families on therapy goals and techniques
Qualifications
- Current California Physical Therapist license
- Strong communication and teamwork skills
- Experience in skilled nursing preferred, but new graduates are welcome to apply
