Overview

The Community Health Worker is a trained community member to provide primary health care referrals including behavioral health and case management, enrolling individuals into medical insurance, and performing community health and SDOH screenings. The CHW will also provide HIV, HEP C , and STI screening, testing, and education presentations.

Responsibilities and Essential Functions
  • Responsible for connecting and establishing relationships with patients and their families to provide support and encouragement to a healthy lifestyle
  • Perform community health screenings and provide appropriate referrals including behavioral health, case management, and other referrals as needed
  • Provide ongoing follow-up encounters, basic motivation interviewing, and goal settings on care and case management plans
  • Engage with patients via phone, home visits, and other community locations where patients can be found
  • Enroll individuals into health insurance as appropriate
  • Act as a patient advocate and liaison between the patient and community service agencies
  • Collaborate and engage in community meetings to connect with other community officials and organizations
  • Provide appropriate referrals including behavioral health, case management, and other referrals as needed
  • Facilitate access to healthcare services for individuals, particularly those with disabilities or chronic illnesses.
  • Provide education on health topics, including nutrition, public health initiatives, and available community resources.
  • Conduct outreach activities to raise awareness about health programs and services.
  • Attend health fairs and provide on-site testing.
  • Collect and analyze data to assess community health needs and program effectiveness.
  • Deliver presentations and workshops on various health-related topics to community groups.
  • Other duties may be assigned.
Qualifications

Education & Experience

Must be a certified Community Health Worker or become a Certified Community Health Worker within 90 days of employment. Community Health experience desired.
