Overview
Oncology Clinic Nurse Specialist role at Beacon Health System. $5,000.00 Sign On Bonus. Reports to the Manager or designee. Manages resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients. Serves as a hospital and Beacon Medical Group ambassador from the patient's first contact to follow-up post-treatment for coordination of care. Provides education regarding services available to oncology patients through and among the different specialty departments of the Cancer Center.
Develops survivorship care plans for all eligible oncology patients and provides teaching and resources for optimal survivorship care.
- Coordinate oncology patient care throughout the continuum of their care by assisting adult oncology patients as they navigate through the healthcare system.
- Conduct initial discussions with newly diagnosed patients (and families) to determine needs or barriers that may influence participation in care.
- Network with physicians and other healthcare providers to ensure effective implementation of patients plans of care and desired patient outcomes.
- Act as a liaison between family and the interdisciplinary team by educating and assisting with understanding of the plan of care for patients, families and healthcare team members.
- Provide support to patients with issues such as insurance, transportation, child care, financial resources, language barriers and education; manage patient concerns so they can focus on getting well.
- Initiate referrals within Beacon Medical Group and community agencies to connect patients with resources to eliminate barriers to care (e.g., pain management, financial counselors, transportation, language barriers, child care, nutritional referrals) to help patients focus on getting well.
- Collaborate with community agencies to coordinate referrals as needed.
- Utilize interdisciplinary care conferences to plan interventions for patients.
- Encourage patient compliance with medical instructions and referrals by educating the patient and family and supporting the physician s treatment recommendations.
- Report information regarding variance trends to the manager/director or appropriate Physician reviewer for review and action.
- Coordinate gathering and reporting of patient outcome information, including follow-up after procedures.
- Develop and discuss survivorship plans outlining treatment received, late/long-term effects and follow-up guidelines.
- Conduct health screenings and provide educational offerings in the community.
- Maintain an active role in the Oncology Care Committee, reporting activities to the group and the American College of Surgeons as required.
- Compile and analyze data required for reports; ensure data represent utilization trends and patterns.
- Prepare summaries and reports for review by the Oncology Care Committee.
- Manage patients under each oncology clinic or Beacon Medical Group clinic.
- Provide direct nursing care for multidisciplinary clinics.
- Coordinate scheduling of necessary procedures and follow-up.
- Perform other functions to maintain personal competence and contribute to department effectiveness (complete other assignments and special projects as directed).
- Identify patients eligible for treatment summaries and survivorship care plans using Cancer Registry data.
- Develop survivorship care plans for all eligible patients.
- Deliver and discuss survivorship care plans with patients and families.
- Attend and participate in department meetings; be accountable for information shared.
- Complete mandatory education, annual competencies and department education within time frames.
- Maintain license/certification, and comply with required health requirements.
- Maintain current licensure and certifications; comply with BCLS/CPR and position requirements.
- Adhere to universal precautions, protective equipment, and ergonomic techniques.
- Adhere to regulatory requirements, surveys, and compliance; comply with policies.
- Be available to work overtime or additional shifts as required.

RN license in Indiana and Oncology Certified Nursing (OCN) certification required; if not certified, certification required within two years of hire. Minimum five years of oncology nursing experience preferred. Leadership experience in healthcare required.
