About the Role:
We are currently seeking a compassionate and skilled Registered Nurse (RN) with hospice or related experience for a full-time, in-person position in South Bend, IN
. This is a direct hire opportunity through EXAR Healthcare Staffing Agency on behalf of a client committed to quality end-of-life care.
As a Registered Nurse in this role, you’ll work closely with interdisciplinary teams to develop personalized plans of care, provide direct patient support, and advocate for your patients’ comfort and well-being.
Key Responsibilities:Coordinate with care team members to assess, plan, implement, and evaluate patient care
Create and manage individualized care plans based on clinical assessments
Administer medications and monitor patient response
Observe, document, and report changes in patient condition
Maintain accurate and timely records in compliance with nursing standards
✔ Active Registered Nurse (RN) license in the state of Indiana
✔ Minimum 2 years of nursing experience (hospice or home health experience strongly preferred)
✔ Strong clinical skills with high emotional intelligence
✔ CPR and First Aid certified
✔ Valid driver’s license and reliable transportation
✔ Strong communication and patient advocacy skills
