⭐About the Role
The Registered Nurse Case Manager – Hospice is responsible for coordinating and delivering high-quality hospice nursing care in the home setting
, ensuring each patient receives a personalized plan of care tailored to their medical, emotional, and spiritual needs. You will collaborate closely with interdisciplinary teams—including physicians, social workers, chaplains, CNAs, and therapists—to ensure seamless, holistic care.
This is a field-based role requiring strong clinical judgment, excellent communication skills, and the ability to work independently while being fully supported by a collaborative leadership team.
Key Responsibilities- Conduct comprehensive in-home patient assessments and develop individualized hospice care plans
- Provide direct nursing care, including medication administration, symptom management, wound care, and palliative treatments
- Coordinate care with physicians, hospice medical directors, social workers, chaplains, CNAs, and therapists
- Monitor changes in patient condition and adjust care plans accordingly
- Educate patients and families on disease progression, medications, symptom control, and comfort-focused care
- Support families with emotional guidance and preparation during the end-of-life process
- Document all patient visits, clinical notes, and care updates in compliance with hospice regulations and timelines
- Participate in on-call rotation as needed to ensure patients receive timely support
- Advocate for patient needs and maintain the highest standards of hospice nursing practice
- Active Registered Nurse (RN) license in the state of Indiana
- Minimum 1 year of nursing experience; hospice or home health experience strongly preferred
- Strong assessment skills, clinical judgment, and the ability to respond quickly to patient changes
- Excellent communication and organizational abilities
- Reliable transportation, valid driver’s license, and the ability to travel within the service area
- Current BLS certification
