• Travel assignment in a long-term care setting

• Provide direct patient care to residents under supervision of a registered nurse in a long-term care setting
• Administer medications and track treatments per orders and facility protocols
• Monitor resident status, perform routine assessments, and document findings
• Assist with activities of daily living (ADLs) and promote resident independence
• Maintain accurate, timely documentation in the chart/EHR and communicate changes to the care team
• Uphold safety, infection control practices, and care planning participation; collaborate with interdisciplinary teammates

• Active Licensed Practical Nurse license (VT) or eligibility to practice in Vermont
• Current BLS/CPR certification
• Prior experience in long-term care or geriatric settings preferred
• Ability to work 40 hours per week; available for day and/or evening shifts
• Strong communication, teamwork, and critical-thinking skills
