Seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a contract opportunity in a long-term care and skilled nursing setting. The LPN will provide direct patient care, administer medications and treatments, monitor resident conditions, and collaborate with the healthcare team to deliver exceptional resident-centered care.

REQUIREMENTS
  • Active Licensed Practical Nurse (LPN/LVN) License in good standing
  • Current BLS certification preferred
  • Previous long-term care, skilled nursing, rehabilitation, or related healthcare experience preferred
  • Strong clinical assessment and documentation skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively within an interdisciplinary healthcare team
KEY RESPONSIBILITIES
  • Provide direct nursing care and treatments to residents
  • Administer medications and monitor resident responses
  • Assess resident conditions and report significant changes to the healthcare team
  • Maintain accurate and timely clinical documentation
  • Assist in the development and implementation of individualized care plans
  • Collaborate with physicians, nurses, therapists, and support staff
  • Educate residents and families regarding care plans and treatment goals
  • Promote resident safety, dignity, and quality of life
  • Ensure compliance with facility policies, procedures, and regulatory standards
ADDITIONAL INFORMATION
  • Guaranteed 40 hours per week
  • Competitive pay rate of $36.00 - $42.00 per hour
