Position: PRN LPN Needed ASAP for Clinic in St. Matthews, SC!
Location: Saint Matthews

ESSENTIAL JOB TASKS:

  • Assess and monitor patients' conditions; conduct health risk assessments.
  • Administer and reconcile medications, treatments, and immunizations prescribed by the provider. Call prescriptions into the pharmacy as directed. Document all OTC medications, supplements, and alternative therapies. Provide information on new prescriptions.
  • Ensure medication list is current in EHS.
  • Provide and document health behavior counseling.
  • Prepare patients for treatments and diagnostic procedures; administer or arrange for diagnostic tests and treatments as instructed.
  • Obtain specimens for laboratory analysis; perform tests as appropriate; notify patients of lab and radiology results as directed; track results.
  • Coordinate patient panels and address care gaps for the clinical team.
  • Make and track all referrals, including community resources as directed.
  • Maintain accurate and complete patient medical records and other documentation.
  • Provide information, instruction, and counseling on health maintenance, promotion, social issues, illness prevention, management, and medication use.
  • Follow proper procedures for handling and disposing of biohazardous waste.
  • Comply with infection control and safety procedures.
  • Maintain quality control of in-house waived testing equipment, monitor refrigerator temperatures, and perform other duties as necessary.
  • Sterilize equipment according to protocol and document the process.
  • Participate in daily team pre-visit planning (e.g., huddles).
  • Order and stock supplies for the team.
  • Adhere to FHC’s corporate compliance policies.
  • Interact respectfully, courteously, and responsively with patients, visitors, and co-workers.
  • Perform other duties and responsibilities as assigned or delegated.
  • QUALIFICATIONS:

    Completion of an accredited LPN nursing program and South Carolina nursing licensure are required. Two years of nursing experience in a healthcare setting is preferred. Current CPR certification is required. Experience with electronic medical records is preferred.

    KNOWLEDGE, SKILLS, AND ABILITIES:

    Must have the ability to write clearly, work cooperatively with patients and staff, and maintain integrity and confidentiality. Basic knowledge of computer applications, including electronic medical records, is necessary. Ability to work under pressure is essential.

    PHYSICAL REQUIREMENTS:

    Must be able to communicate effectively in the region's dominant language, lift 30 lbs, and have normal vision and hearing (corrected if necessary). Independent mobility is required.
