About the Role:As a Registered Nurse (RN), you will be responsible for providing high-quality patient care in our state-of-the-art skilled nursing facility. Your primary focus will be on assessing, planning, implementing, and evaluating individualized patient care plans to ensure the optimal health and well-being of our residents. You will work closely with our interdisciplinary team of healthcare professionals to deliver exceptional care and support to our patients.
Responsibilities:- Assess patients' physical, emotional, and psychological needs, and develop individualized care plans to address these needs
- Implement and evaluate patient care plans, making adjustments as necessary
- Administer medications and treatments, perform routine tests and procedures, and provide wound care
- Monitor and report patient progress to the healthcare team
- Provide education and support to patients and their families to promote healthy living and disease prevention
