What You'll Do:
- Assist with personal care and activities of daily living
- Support safe mobility, transfers and independence
- Prepare meals and provide light household support as assigned
- Offer companionship and respectful emotional support
- Follow the care plan and report changes or concerns promptly
- Communicate professionally with clients, families and the Home Well team
