Responsibilities- Provides a comprehensive assessment of a patient’s psychosocial needs and develops treatment and review plans according to department policy.
- Provides effective behavioral/medical health treatment and education, including assessment, psychotherapeutic counseling, or therapy (individual and/or group), and interprets information to staff, families, and/or agencies.
- Completes all documentation in accordance with department and hospital policy.
- Provides all services and interventions in accordance with licensure regulations (NASW or MFT).
- Utilises patient information to determine needs, grow, develop and understand evidence‑based treatment options for specific populations.
- Provides comprehensive case‑management services/assessment as warranted.
- Assesses Social Determinants of Health and provides relevant information, education and/or referrals.
- Collaborates with patient, family and support persons to develop a comprehensive discharge plan.
- Reassesses treatment plan regularly, evaluates progress, revises plan when clinically indicated.
- Maintains protected patient health information as required by law, state licensing, TJC and HMH policy.
- Assists behavioral/medical team members in responding to emergencies.
- Has a working knowledge of community agencies and resources, referring patients appropriately.
- Lifts and pushes/pulls at least 5 lbs., and stands for minimum 4 hours a day.
- Identifies patient population needs and adapts care accordingly (age, culture, language, etc.).
- Adheres to HMH organizational competencies and standards of behavior.
- Receives proper supervision as per licensed board.
- Performs additional duties as assigned.
- Master’s degree in Social Work (MSW).
- 1 year of total work experience in Social Work.
- Ability to perform psychosocial assessments and develop treatment plans.
- Knowledge of crisis intervention theory and its application.
- Knowledge of disease management, disease process, differential diagnosis and treatment techniques.
- Ability to work as part of a team and build collaborative relationships.
- Excellent written and verbal communication skills.
- Proficient computer skills (Microsoft Office, Google Suite).
- Previous home health/hospice experience.
- NJ Licensed Clinical Social Worker.
Minimum rate of $42,640 annually. Posted rate is an estimate of minimum base pay. The base compensation may vary based on market data, experience, education, certifications, skills, geographic location, internal equity, and budgeting.
