Horizon Health is seeking applicants for a full‑time Care Coordination Manager. The Care Coordination Manager is responsible for leading and overseeing the care coordination team to ensure high‑quality, patient‑centered care. This role involves supervising nursing & non‑nursing staff, developing workflows and procedures, collaborating with interdisciplinary teams, and optimizing care transitions to enhance patient outcomes. The Care Coordination Manager will also be responsible for compliance with healthcare regulations, staff development, and operational efficiency within the care coordination department.

Successful candidates for the Care Coordination Manager should be passionate about providing high‑quality, patient‑centered are.   • Oversee the development and implementation of workflows and procedures for the care coordination & clinical teams.
  • Supervise, mentor, and support care coordination team members.
