Key Responsibilities
  • Provide primary health care to patients, according to established evidence‑based guidelines of clinical practice.
  • Lead care teams to provide high‑quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
  • Manage long‑term care for acute and chronically ill patients, including care coordination, development of care plans, and self‑management plans.
Qualifications
  • Must be licensed or able to obtain licensure in the State of South Dakota.
  • Certification from an accredited organization is required or must be obtained prior to employment.
  • New graduates are encouraged to apply.
