Position: Youth Case Manager/ After Care
Location: Eagle ButteSummary of Position
The primary purpose of the Youth Case Manager is to enhance the quality of the group care resident services and to promote continuity of care through case management for each youth resident of Sacred Heart Center Child Services. This is a licensed group care facility for minors placed by child protection agencies. This position is a program manager and will assist with program development and implementing programming and services for the Sacred Heart Child Services department including casement.
This is a non- exempt position.
AccountabilityReports to the Youth Case Manager
ResponsibilitiesResponsible for case management services at Child Services/adolescent group residential program, including ensuring compliance with all regulations and licensing and contract requirements and all program policies and procedures.Shift work when needed to meet licensing requirements for staff to resident ratio.Complete daily records through logging interactions with residents, stakeholder, parents, and others related to the care of the residents.Assist residents in the development of his/her treatment plan and case plan by helping youth residents to articulate goals and provide necessary information in developing and achieving treatment plan goals.Responsible for developing, implementing, monitoring, and updating youth resident’s assessments, treatment plans and progress reports.Responsible for coordinating and tracking the delivery of services, treatment referrals, linkage to family, referring agency caseworker and family.Maintain a case management record/file for youth/clients which will include required program and case documentation per contracts; treatment plans assessments, treatment/service plans, progress notes, discharge/termination summaries, etc.Coordinate, facilitate and participate in weekly/bi-weekly team meetings and quarterly treatment team meetings; coordinate cases with treatment team members, placing agencies, counselors, other service providers and attorneys/prosecutors as needed, as well as case plan review meetings with placing agencies, clients, families, and treatment team members.Coordinate with staff and counselors in identifying and arranging educational groups and classes such as independent living skills, anger management, behavior management, etc.Coordinate and assist with preadmission process, orientation, client intakes and assessment of newly admitted youth residents.Responsible for the Cans (Child and Adolescent Needs and Strengths) and ACES (Adverse Childhood Experiences) reporting to the Department of Social Services upon intake and discharge.Facilitate meetings with agencies based on client’s needs. (e.g. Schools, Behavioral Health, Job Corps, Independent Living Skills/ Facilities, treatment centers, etc.) And provide advocacy on behalf of each youth resident in securing needed services and resources.Be part of a development team in facilitating and assisting in organizational planning and development of programming and services including assisting needs assessment (program and community), assisting in planning, development and implementation of special projects including family violence, domestic violence and sexual assault programming and services.Assist with the planning and development of outreach resources and activities including prevention programming and activities.Provide aftercare services through the following requirements.Provides other services and work duties as needed to meet the needs of the Sacred Heart Center and its ability to carry out its mission.Other DutiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. Qualifications- Bachelor’s degree from a Regionally Accredited institution in behavioral or social science, or closely related discipline.
- Master’s degree in appropriate discipline is preferred.
