Occupational Therapy + Care Coordination:
The Best of Both Worlds! We’re looking for Occupational Therapists to support our innovative new care coordination program. This opportunity empowers you to practice at the top of your license, while giving you the chance to branch out into care coordination duties designed to standardize your hours and grow your career. • Providing occupational therapy and wellness services for older adults
• Conducting assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team
• Supporting care coordination efforts by conducting regular wellness checkpoint visits, communicating with patients’ providers and caregivers, and assisting patients with telehealth visits
• Maintaining professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
• Contributing to program development, quality improvement, and problem-solving
• Optimizing each patient’s functional well-being and satisfaction

Our clinics typically operate 8 a.m. to 5 p.m. Monday through Friday, with flexible scheduling to fit your lifestyle.

Requirements include:

• Completion of an accredited Occupational Therapy program (new grads welcome)
• Current state licensure in OT
• Excellent verbal and written communication skills
• Results-oriented mindset with critical thinking
• Tech-savvy; quick learner
• Professional curiosity and problem-solving ability
