Hospice CNA
We are seeking a self motivated, patient centered Hospice Aide to join our team at Ovation Hospice. Come join a team of dedicated, smart, and caring professionals as they work together to make sure our patients come first, always.
Role Overview
The hospice aide is responsible for providing personal care under the direction of a registered nurse. The expected hours of work are 5-10 for tuck ins and availability for 7-2 for am shifts. We are in need of part time employee with possibility of becoming full time.

Responsibilities
- Document care provided according to organization policy.
- Report any deviations from the Plan of Care to the Case Manager
- Use Durable Medical Equipment safely including hoyerlift and sit to stand.
- Follow instructions provided by RN case manager via Hospice Aide Plan of Care.
- Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care any by active participation in team and staff conferences.
- Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
- Assist with ambulation, transfer, range of motion if documented on the Hospice Aide Plan of Care.
- Participate in QAPI activities.
Qualifications
- Hold Active CNA License in the state of Utah
- One year experience as a CNA, hospice aide or home health aide.
- At least 18 years old.
- Must be a licensed driver with an insured automobilein good working order.
- The ability to communicate well, both verbally and in writing.
- Ability to work as a team and also independently
