Bilingual Social Worker (Part-Time)

Families First Pediatrics | West Jordan, UT, serving our southern clinics
Status: Part-Time, Non-Exempt | 20-25 hours per week

About the Role

Families First Pediatrics is looking for a Bilingual Social Worker to join our care team. You'll work alongside our medical providers to identify patients and families who need help with medical coverage, behavioral health care, food, housing, transportation, and other community services, and then help them get connected. Service is delivered directly in Spanish, not through an interpreter.

This is an entry-level social work position under general supervision, focused on case management, screening, advocacy, and supportive contact. It does not involve diagnosing or treating mental illness or providing psychotherapy. Clinical concerns go to a licensed therapist or the provider.

What You'll Do

Case management and navigation

  • Meet with referred patients and families, in person or by phone, to understand their needs and barriers
  • Screen for unmet needs, including coverage, behavioral health, food, housing, transportation, childcare, utilities, immunization access, and school-related services
  • Complete and submit applications with families (Medicaid, CHIP, the Marketplace, WIC, SNAP, and clinic financial assistance) and track each one to a decision
  • Place referrals to community agencies and behavioral health providers, and confirm the patient was reached
  • Follow up on open cases on a set schedule and stay current on agencies, programs, and eligibility rules in our southern service area

Language access

  • Deliver the full service in Spanish, including applications, calls, and written follow-up
  • Interpret between Spanish-speaking families and providers during case management conversations
  • Adapt Spanish patient materials so they read naturally, not as literal translations

Working with providers and clinic teams

  • Take referrals from providers and clinic staff, respond within the agreed turnaround, and report back on what was arranged and what remains open
  • Attend clinic huddles or care team meetings at southern sites on an agreed rotation
  • Raise patterns, such as a closed program or a recurring payer denial, rather than absorbing them case by case

Records, confidentiality, and safeguarding

  • Document every contact, referral, and application in the medical record promptly and factually
  • Handle patient information in compliance with HIPAA and the added protections for behavioral health and minor patient information
  • Follow mandatory reporting obligations and elevate safety concerns to the provider and Director without delay
  • Work within license boundaries and refer anything beyond them
Qualifications

Required

  • Bachelor of Social Work from a CSWE-accredited program
  • Utah Social Service Worker (SSW) license, or eligibility to be licensed, with licensure obtained within 90 days of hire. A CSWE-accredited BSW qualifies for the SSW with no examination and no additional supervised hours.
  • Full professional fluency in Spanish and English, spoken and written. Fluency is tested s is a requirement, not a preference, because the position serves Spanish-speaking patients and families directly.
  • Valid driver license, reliable transportation, and current automobile insurance, as the role travels between southern clinics
  • Sound judgment about what belongs in the record, with the provider, or with a licensed therapist, and discretion with confidential patient and family information

Preferred

  • Working knowledge of Medicaid, CHIP, and community assistance programs in Utah, or the ability to learn them quickly
  • Experience in a healthcare, public health, or community services setting
Schedule and Work Environment
  • 20 to 25 hours per week, set according to clinic need and subject to change
  • Occasional hours outside the standard clinic day when a family cannot meet during it
  • Clinic and office setting, with patient contact in exam rooms, offices, and by telephone
  • Extended time at a workstation and on the phone
  • Regular travel between southern clinics during the workday, with mileage reimbursed at the current rate
