The Registered Nurse (RN) works with all members of the Patient-Centered Medical Home (PCMH) Care Team by performing a variety of clinical and administrative patient‑related duties as established in the roles and responsibilities defined below. The RN actively participates in team huddles and care team meetings. The RN provides professional nursing care, that is safe and effective, utilizing established policies and procedures.
The RN assesses, plans, implements and evaluates care in collaboration with physicians and practitioners. The RN provides leadership and supervision to all clinical staff. The RN coordinates department activities, ensuring safety of patients and staff and ensures that adequate medical supplies, medication and vaccinations are ordered and are on the premises. Services are provided based on the ages of the population served, newborn through geriatric (65 years or older), in collaboration with the care team.
Education
- Status:
Must be a graduate from an accredited school of nursing with a current Registered Nurse license specific to the State of employment - Preferred:
Bachelor’s degree in Nursing (BSN)

Two years of nursing work experience - Preferred:
Ambulatory care/physician office experience - Preferred:
Prior experience with chronic disease management and PCMH - Alternative:
If applying for UC/Occupation Health Services – Emergency room or Urgent Care experience preferred
- Intensive knowledge of a highly specialized field or broad knowledge of major hospital and PCMH functions or activities
- Extensive knowledge of epidemiological principles, infectious/communicable diseases, health assessment, health management, and adult immunizations
- Highly effective written and verbal communication skills
- Excellent interpersonal skills
- Proficient computer skills as required to document, communicate and enter information into the electronic medical records system
- Patient safety compliance and effective communication in English; bilingual abilities preferred
- Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date
- For Urgent Care/Occupational Health Services positions – Current ACLS card required; must complete course within 2 weeks of hire
- Current California Nursing License in good standing (California position)
- For Nevada position – Current California and Nevada Nursing licenses in good standing required
- Walk, stand, sit, talk, or hear frequently
- Use hands for handling tools/controls; reach with hands and arms occasionally; climb, balance, stoop, kneel, crouch, or crawl occasionally
- Specific vision abilities: close vision, color vision, ability to adjust focus
- Lift, push, transfer patients; occasionally move up to 50 pounds
