An exciting opportunity has arisen within Clydesdale in Adult and Older People's Services. We are looking to appoint a motivated and enthusiastic permanent full-time Social Worker in our in our Older Person’s team.

Clydesdale locality within South Lanarkshire covers the largest geographical area of 1,326.31 square kilometres (512.10 square miles) of the four Locality Area’s. Although Clydesdale has several urban centres such as Lanark, Carluke, Lesmahagow, Biggar, Forth and Douglas it is the most rural area of South Lanarkshire, where issues such as transport, access to services and rural deprivation can have a marked effect.

Clydesdale has a population of 61,671 of which 27% are aged 65 and over, with a higher percentage of aged 75 and over (9%) in comparison with the rest of South Lanarkshire which sits at 8.3%.

Given the local geography and demographic factors, Health and Social Work teams in Clydesdale were already working in partnership prior to the implementation of Health and Social Care Integration. As such we have long established professional multi-agency working networks and processes that support our person-centred approach to supporting our vulnerable citizens.

Purpose of the job

To provide a professional level of service to the identified client groups in a multi-disciplinary setting under minimal supervision following the guidance and instruction of team leader/manager. This work is to be carried out within the framework of relevant legislation and in accordance with Social Work policy.

Key tasks and responsibilities

To provide an assessment and care management service including those cases where there is a statutory duty such as Adult Support and Protection and Adults with Incapacity.

To undertake work with minimal guidance, evidencing ability to act effectively as a case manager and undertake work where there is multiple agency involvement.

Undertaking and/or overseeing major pieces of casework where there is major significant degree of risk such as risk of personal harm to the service user, risk of harm to others and/or public safety issues. Direct intervention will be required in high-risk situations where there is statutory duty.

To provide professional advice and guidance for individuals, families and groups within the community working in partnership with appropriate agencies on behalf of clients where this is required.

Complete all professional, formal and informal reports and all written case notes timeously and to the highest professional standards including updating all relevant IT systems where appropriate.

Undertakes work that demonstrates practice informed by an evidence-based practice agenda and uses underpinning knowledge to evaluate outcomes for service users in relation to social work interventions.

To contribute to the review process for service users.
