As the Clinical Support Coordinator reporting directly to the Director of Nursing, you will be a coach, mentor, and sometimes supervisor for others in the Care Giving Department in the absence of a licensed nurse and Director of Nursing. In addition, you will provide support and direct care to the residents in activities of daily living, fostering independence, and promoting individuality, while respecting and protecting each resident’s right to privacy.
Essential Duties and Responsibilities- Responsible for creating the Caregiver schedule, consulting with the Director of Nursing when necessary for guidance. Point person for scheduling-related questions (includes being on-call for scheduling issues).
- May supervise caregivers when the licensed nurse or Director of Nursing is unavailable.
- Participates in competency training of new associates; overseeing the orientation and onboarding process. Conducts ongoing training for caregiving associates.
- Works collaboratively to problem solve and address issues with new caregiver associates in a timely manner.
- Make physician appointments, therapy appointments and arrange for transportation to appointments as needed.
- Assists residents with Activities Daily Log (ADL) per Individualized Service Plan.
- Attends to the individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating and/or other needs in keeping with the Individualized Service Plan requirements.
- Supervise and assure completion of any required documentation in the resident’s medical record.
- Supports resident’s participation in scheduled and unscheduled activities.
- Reviews and follows all instructions on the resident’s medical record.
- Uses good body mechanics and monitor environmental factors for potential safety hazards daily.
- Maintains clean, orderly and safe environment for residents and complies with and monitors adherence to infection control and medication policies and procedures.
- Informs the Director of Nursing, or other appropriate staff regarding changes in resident physical, emotional or behavioral status, orally and via the communication log.
- Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia; promptly reports missing items according to the Company’s policy and participates in efforts to locate missing items.
- Other duties may be assigned.
Acts as Lead Caregiver under direct instruction of the Director of Nursing. Carries out supervisory responsibilities in accordance with the Company policies, the Code of Ethical Business Conduct and applicable laws.
Requirements Education- Associate’s or Bachelor’s degree or equivalent from a college or technical school
- Education in gerontology, dementia care, recreation therapy, social work or extensive professional experience preferred
- Collaborative team player able to establish effective working relationships with team members.
- Demonstrated experience in promoting a high level of customer service and hospitality towards and among all residents, associates, and visitors.
- Possess sound organizational skills with the ability to multi-task and with a strong attention to detail.
- Self-motivated to accomplish goals with a strong sense of accountability for results.
- Demonstrated communication excellence, verbally and in writing; comfortable presenting to large groups.
- Described as dependable with a strong work ethic.
- Possess an appreciation of seniors and an understanding of the aging process.
- Basic knowledge in Microsoft Word, Excel and internet browsers are required.
Travel is not required.
