We are glad to share an exciting opportunity to join our Virtual ward (Hospital at Home team) as a coordinator/practitioner. This post’s key responsibilities are care coordination and discharge planning, providing clinical leadership, supporting the service development, supporting patients and families in digital and remote monitoring and supporting development of junior staff.

The post holder will be able to work autonomously and provide a high level of expert clinical management and care to patients within the speciality, supporting the referral process from both in and out of hospital.

Virtual wards (Hospital at Home) work in partnership with our Recovery at Home teams are defined as a safe alternative to NHS bedded care, we are looking to expand our virtual ward team in line with the introduction of new clinical and community step up pathways in addition to the established hospital step down pathways. Hospital at home delivers care closer to home for patients, offering personalised care with technology enabled remote monitoring, providing medical and nursing, therapy and pharmacy optimisation and onward discharge referrals.

The post holder will work collaboratively with the multi‑disciplinary team. The post holder will be expected to cover days and weekend shifts. There will also be dedicated allocated time for management and continuous professional development.

Main duties of the job

We are looking for a forward‑thinking, well‑organised and motivated individual with a positive attitude, and a strong interest in community delivered care. The applicant will need to act as a role model delivering excellent clinical practice and promoting clinical effectiveness in care.

As Hospital at Home Coordinator/Practitioner you will be able to assess, diagnose, and manage a variety of conditions, within a multidisciplinary and collaborative environment.

  • Identify patients suitable for the Hospital at home pathways ensuring timely access to the hospital at home team, either discharged from Acute hospitals or step up from community.
  • Act as a single point of contact for patients and health professionals to facilitate rapid access to Hospital at home services.
  • Work closely with other healthcare professionals in a collaborative, team‑based approach to discharge to assess.
  • Strong clinical background with advanced assessment skills.
  • Contribute to improving patient outcomes and service delivery within the community services.
Job responsibilities

Lead the development of integrated, patient‑centred health and social care pathways across agencies and disciplines.

Act as a clinical leader and patient advocate, promoting risk management, self‑care, and strategic planning.

Support and train junior staff to deliver high‑quality care within the Hospital at Home service.

Work collaboratively across multi‑professional teams to ensure safe, effective, and efficient practice.

Maintain and share up‑to‑date clinical expertise to continually enhance service quality and patient outcomes.

Qualifications
  • Degree in a health‑related subject and current professional registration (NMC or HCPC).
  • Advanced physical assessment and history taking course (accredited).
  • Masters in health / leadership related subject.
  • Management and leadership training.
Physical Skills
  • Ability to assist patients to move as per Trust Moving and Handling Policy.
  • Car Driver (full license) and access to car for work purposes.
Experience
  • Extensive post registration experience, some of which will be at a senior level within the relevant speciality.
  • Portfolio of evidence of continuous professional development.
  • Experience of practice development, clinical education or leading change.
