To plan, set up, and deliver outreach NHS Health Check clinics within local communities, particularly targeting underserved or hard-to-reach groups.
The post holder will work closely with GP practices to identify eligible patients to be invited to the outreach events.
The post holder will work with local public health teams, and community organisations to increase awareness, access, and uptake of NHS Health Checks.

STHC is a not-for-profit GP Federation, set up in 2017, to bring together GP practices to work collaboratively, delivering services at scale across the borough of South Tyneside and is formed from 18 of the 21 practices across South Tyneside.
