An exciting opportunity has arisen for an enthusiastic, well-motivated and dedicated Physiotherapist to join our Recovery at Home Team, South Tyneside.
This is a unique and rewarding role which will allow you to work with, lead and support colleagues during a time of innovation and development as our team focuses on the implementation of Urgent Community Response and of Virtual Wards.
You will work within Directorate One of Community Services, supported by senior colleagues who are passionate and proud of the services we deliver to our community of South Tyneside and Sunderland.

Main duties of the jobThis role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short-term support for their physical and functional health needs to remain at home.
The service is fast paced, and you would be expected to see a wide range of patients who present with a variety of conditions.
This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills.
Please note a Full UK Driving Licence is essential for this role.

- Registered with the HCPC
- Portfolio of evidence of competence and continuous professional development
- Be able to undertake comprehensive holistic assessment of patients, be able to prioritise clinical need and implement a plan of care reflecting evidence-based guidance
