We're looking for a Living Made Easy Assessor to join our South Tyneside team. This is a varied role, working equally between our branch and out in the community, supporting people to remain as independent and safe as possible in their own homes.
This role is funded by the Local Authority but employed by Human Support Group, and you'll work closely with both our Registered Manager and the Local Authority's Single-Handed Care Lead.
Job Description What you'll doYou'll work directly with people who are new to our reablement service, helping them identify practical ways to regain or maintain their independence.
Your Day Could Include- Visiting people in their own homes to complete assessments following their referral to our reablement service.
- Assessing daily living, mobility and moving and handling needs.
- Identifying practical solutions and recommending appropriate equipment to help people remain independent.
- Once fully trained and assessed as competent, ordering low-level equipment such as commodes, walking frames, bed levers and other aids.
- Visiting existing service users where moving and handling concerns or other issues have been identified and finding safe, practical solutions.
- Recognising when a person's needs are more complex and making appropriate referrals to services such as Recovery at Home or other support networks.
- Working with individuals, families and informal carers to understand what matters to them and what they want to achieve.
- Supporting people to develop their daily living skills and maximise their independence.
- Completing accurate assessments and providing these to the Local Authority within agreed timescales.
- Reviewing accidents and incidents recorded within our internal system, identifying themes and trends, making referrals and escalating concerns where appropriate.
- Attending meetings, networking sessions and additional training with the Local Authority and our internal team.
- Working collaboratively with Social Care, Health and other professionals to ensure people receive the right support at the right time.
You don't need to be an occupational therapist to succeed in this role.
The successful candidate will complete specialist training with the Local Authority and will be assessed as competent before carrying out equipment assessments and ordering appropriate low-level equipment.
You'll also develop your knowledge of moving and handling, reablement, strengths-based approaches and supporting people to achieve greater independence.

- Experienced in health, social care, rehabilitation, reablement, moving and handling or a related field.
- Confident communicating with people, families and other professionals.
- A good listener who can understand what matters to the individual rather than simply focusing on what they can’t do.
- Comfortable working independently in people’s homes and making appropriate decisions.
- Organised and able to complete accurate assessments and documentation within required timescales.
- Able to recognise risk and know when further professional support is required.
- Passionate about promoting independence, choice and dignity.
- Comfortable working collaboratively across different teams and organisations.
- Confident using IT and maintaining accurate records.
