We are excited to offer an opportunity for an experienced and motivated Advanced Nurse Practitioner to join our friendly and supportive team. Main duties of the job

The ANP will work autonomously and as part of the wider team to deliver high-quality care, including:

  • Assessment, diagnosis, and management of patients with acute and undifferentiated presentations
  • Management of long-term conditions, including monitoring and actioning routine blood tests
  • Same-day care / triage, supporting patient access to services
  • Prescribing and medication review in line with current guidelines
  • Requesting, interpreting, and acting on investigations (e.g. blood tests, imaging)
  • Providing follow-up and continuity of care for patients
  • Delivering health promotion, preventative care, and self-management support
  • Contributing to service development and quality improvement initiatives

The post holder will also support the practices commitment to Year of Care and personalised care planning, particularly for patients with long-term conditions.

Job Summary

The post holder will be an experienced Advanced Nurse Practitioner working at a Master's level (Level
8), holding or working towards a recognised MSc in Advanced Clinical Practice and associated digital credential/badge evidencing advanced practice capabilities.

They will demonstrate highly developed clinical decision-making, critical thinking, and autonomous practice within defined professional boundaries. The ANP will assess, diagnose, treat, and manage patients presenting with both acute and chronic conditions, ensuring safe, evidence-based, and patient-centred care.

The role includes leadership, service development, and collaborative working across the multidisciplinary team to support high-quality service delivery aligned with national policy and local priorities.

Key Responsibilities

Clinical Practice

Work within a defined scope of practice, with appropriate clinical supervision and governance arrangements.

  • Independently assess, diagnose, plan, implement, and evaluate care for patients with undifferentiated and complex conditions

Contribute to triage systems and improve patient access (e.g. digital consultations, same-day care)

  • Provide expert clinical management of acute and long-term conditions
  • Prescribe safely and effectively in line with prescribing qualifications and current guidelines
  • Initiate appropriate investigations and interpret results
  • Deliver health promotion, prevention, and self-management support
  • Recognise and respond to urgent and emergency presentations appropriately

Monitor and manage patients with long-term conditions, including requesting, reviewing, and actioning routine blood tests in line with agreed protocols

Provide follow-up and continuity of care for complex patients
