Job summary
Ellison View Surgery is seeking an enthusiastic, caring and motivated Practice Nurse to join our friendly and supportive clinical team. This permanent post has arisen as part of our proactive workforce planning following the partial retirement of a long-serving and highly valued member of our nursing team. The role will be available from April 2027, allowing for a planned transition and continuity of excellent patient care.
The role is offered at 17 hours per week (2 Days).
This is an exciting opportunity to become part of an experienced nursing team comprising:
- 1 Advanced Nurse Practitioner (ANP)
- 2 Practice Nurses
- 3 Healthcare Assistants
Working alongside our wider multidisciplinary team, you will deliver high-quality, evidence-based nursing care to our patient population while contributing to the continued development of our services. We are looking for an individual who is committed to delivering excellent patient care, enjoys working collaboratively and is keen to develop within primary care.
Main duties of the job- Providing holistic nursing care to patients of all ages within a primary care setting.
- Managing and reviewing patients with long-term conditions, including diabetes, asthma, COPD, hypertension and cardiovascular disease, in line with current national guidance and individual competencies.
- Undertaking chronic disease monitoring and annual reviews using recognised clinical templates.
- Performing cervical cytology (desirable) in accordance with the NHS Cervical Screening Programme.
- Delivering childhood and adult immunisations, seasonal vaccination programmes (including influenza, COVID-19, pneumococcal, RSV and shingles) in accordance with national immunisation schedules.
- Administering travel vaccinations (where appropriately trained).
- Providing wound assessment, wound care, ulcer management, dressings and suture/staple removal.
- Undertaking NHS Health Checks and cardiovascular risk assessments.
- Providing contraception advice and administering contraceptive injections where competent.
- Supporting patients with health promotion, smoking cessation, weight management, alcohol awareness and healthy lifestyle advice.
- Delivering patient education to encourage self-management of long-term conditions.
Ellison View Surgery is a friendly and forward-thinking GP practice providing high-quality primary care to approximately 6,000 patients. Our experienced multidisciplinary clinical team consists of 3 GP Partners, 1 Salaried GP, 1 Advanced Nurse Practitioner, 2 Practice Nurses and 2 Healthcare Assistants, all working collaboratively to deliver safe, effective and patient-centered care. Our dedicated administration team is made up of skilled and committed Medical Administrators who play a vital role in supporting the smooth running of the practice and ensuring an excellent patient experience.

responsibilities
- Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
- Consult with patients and their families to undertake assessments of patient need
- Support in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives
- Develop, implement and embed an effective care management programme for the frail and housebound patients registered with the organisation
- Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice
- Embed population health management approaches within general practice, targeting health inequalities and improving access
- As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
