.
South Tyneside is a place defined by its people, strong communities, deep pride, and a shared commitment to looking after one another. As the Council continues to transform Adult Social Care, the Assistant Team Manager within the Hospital Discharge Team plays a central role in ensuring that every resident with care and support needs can live a Better Life in the place they call home.
TheImportance of the Role
The Hospital Discharge Team is a critical part of the local health and social care system. The Assistant Team Manager provides leadership at the point where residents are often at their most vulnerable, transitioning from hospital back into the community. The role ensures that support is timely, safe, strengths‑based, and coordinated across partners. Decisions made by the Assistant Team Manager directly influence a person’s recovery, independence, and long‑term wellbeing.
The position also includes deputising for the Team Manager in their absence, ensuring continuity of leadership, decision‑making, and operational oversight during periods of leave or service demand. This responsibility highlights the seniority and influence of the role within the wider service. This is a position where leadership has immediate, visible impact. Supporting a resident to return home safely, regain confidence, or feel heard during a difficult moment brings a level of professional reward that few roles can match.
