Importance of the Role

The Hospital Discharge Team is a critical part of the local health and social care system. The Assistant Team Manager provides leadership at the point where residents are often at their most vulnerable, transitioning from hospital back into the community. The role ensures that support is timely, safe, strengths‑based, and coordinated across partners. Decisions made by the Assistant Team Manager directly influence a person’s recovery, independence, and long‑term wellbeing.

The position also includes deputising for the Team Manager in their absence, ensuring continuity of leadership, decision‑making, and operational oversight during periods of leave or service demand.   • Professional qualification and registration as a Social Worker
  • Strong understanding of legislation, safeguarding, and best practice
  • The ability to supervise, motivate, and develop staff
