About the role

This is an exciting opportunity to join our Hospital Discharge Team, playing a vital role in supporting adults with care and support needs to achieve the outcomes that matter most to them. Working in a fast-paced and rewarding environment, you will undertake a range of assessments and interventions, working closely with individuals, families, carers, health colleagues and partner organisations to support safe, timely and person-centred hospital discharges.

You'll be at the heart of delivering our Home First and Discharge to Assess approach, ensuring people spend no longer in hospital than necessary and are supported to recover, regain independence and remain connected to their communities. Rather than making long-term decisions in a hospital setting, you'll help ensure assessments take place in the most appropriate environment, focusing on people's strengths, what matters to them, and the support available within their families and communities.

As part of a multidisciplinary team, you will work closely with NHS and partner agencies to facilitate seamless transitions from hospital, reduce delays, promote independence and improve outcomes for local people. This is an opportunity to make a real difference every day by helping people return to the place they call home and supporting them to live as independently as possible.

South Tyneside Council is committed to innovation, partnership working and continuous improvement. We are leading the way in developing strengths-based, person-centred approaches that focus on prevention, recovery and reducing long-term dependency on services. The successful applicants will share our enthusiasm for improving outcomes for people, contributing to service development and influencing performance at an individual, team and service level. In return, you'll benefit from a wide range of tailored Continuing Professional Development opportunities, alongside ongoing support to help you achieve your career aspirations.,

This is an exciting time to join Adult Social Care in South Tyneside. Our vision is to help every person with care and support needs to Live Better Lives by building on their strengths, promoting independence and ensuring they remain safe, well and connected within their communities. You'll be joining a forward-thinking council that places People, Place and Partnerships at the heart of everything we do.

Together with our partners, we're transforming services, embedding strengths-based practice, streamlining processes and developing innovative approaches that improve outcomes for residents. Our colleagues tell us they are proud of the difference they make to the residents of South Tyneside, and we are committed to ensuring that everyone has the support, training and opportunities they need to succeed. We are committed to helping our staff grow and succeed through our CARE Values and Behaviours Framework, empowering colleagues to be:
Curious. Accountable. Responsive. Empowered. Joining us now means being part of an ambitious transformation journey, helping to shape and embed co-production, deliver innovative models of care and support, and influence the future of Adult Social Care in South Tyneside. If you're passionate about supporting people to achieve the best possible outcomes, promoting independence and delivering effective Home First and Discharge to Assess practice, we'd love to hear from you.
