Community Health Worker Facilitator

About the Role

Our client is looking for an experienced and motivated Community Health Worker Facilitator to join their public health team in Nelspruit . This role focuses on training, supporting, and supervising a team of community health workers (CHWs) who deliver essential health services at the grassroots level within the Mpumalanga province. You will be instrumental in enhancing the capacity of CHWs, ensuring the effective implementation of health programs, and improving health outcomes for underserved populations in and around Nelspruit .

• Train and mentor Community Health Workers on health protocols and service delivery.
• Supervise the daily activities and performance of a team of CHWs.
• Ensure the accurate collection and reporting of health data from community outreach.
Facilitate community engagement initiatives and health awareness campaigns.
• Monitor the quality of health services provided by CHWs in the field.
• Liaise with local clinics and health facilities to coordinate care in Nelspruit .

• Diploma or degree in Community Health, Nursing, Public Health, or a related field.
• At least 3 years of experience working in community health, with experience in training or supervision.
• Strong understanding of primary healthcare principles and public health programs in South Africa.
• Excellent facilitation, communication, and interpersonal skills.
• Ability to travel within the Mpumalanga region.
• Proficiency in data collection and basic reporting.
