Elderly Care Coordinator

Our client is looking for a dedicated Elderly Care Coordinator to enhance the quality of life for senior citizens in Nelspruit and surrounding areas. This role involves direct engagement with elderly individuals and their families, coordinating a range of support services, and ensuring their well-being and independence. You will be responsible for assessing needs, developing care plans, and connecting seniors with appropriate resources, including healthcare, social activities, and in-home assistance.

This position is crucial for providing compassionate and effective care within the Mpumalanga community.

• Conduct comprehensive assessments of elderly clients' needs, preferences, and care requirements.
• Develop and implement personalized care plans in collaboration with clients and their families.
• Coordinate a variety of services, including medical appointments, home care, and social engagement.
• Liaise with healthcare providers, community organizations, and support services.
• Monitor the effectiveness of care plans and make necessary adjustments.

• Experience in geriatric care, social work, nursing, or a related field.
