Community Health Nurse

Our client is seeking a dedicated Community Health Nurse to serve the Vereeniging area. This role focuses on promoting health, preventing disease, and providing essential healthcare services directly within the community. You will work with individuals, families, and groups to address health needs, conduct health education, and facilitate access to primary healthcare services. • Conduct health assessments and provide primary nursing care in community settings.
• Implement and manage health promotion programs focusing on areas such as maternal and child health, chronic disease management, and infectious disease control.
• Educate community members on health issues, disease prevention, and healthy lifestyles.
• Facilitate referrals to clinics, hospitals, and other healthcare services.
• Administer immunizations and basic medical treatments as per scope of practice.
• Maintain accurate patient records and contribute to community health data collection.

• Registered Nurse (RN) qualification.
• Current registration with the South African Nursing Council (SANC).
• Minimum of 3 years of experience in community health nursing or a related field.
• Strong clinical skills in primary healthcare assessment and intervention.
• Excellent communication, counseling, and health education abilities.
• Knowledge of public health principles and community-based healthcare models relevant to Vereeniging .
