  • Valid professional registration and authorisation to practise.

Job Description

The Social Worker will provide professional psychosocial and social support to clients and their families. The role will focus on assessing social needs, providing professional social work interventions and supportive counselling within the professional scope of practice, assisting vulnerable individuals and families, coordinating social services, and helping clients access appropriate community resources.

Key Responsibilities

  • Conduct social and psychosocial assessments.
  • Provide professional social work interventions.
  • Offer supportive counselling within the professional scope of practice.
  • Support clients experiencing social, emotional, or family challenges.
  • Assist elderly, vulnerable, and socially isolated clients.
  • Provide family support and facilitate communication.
  • Develop appropriate social support plans.
  • Refer clients to relevant social services and community resources.
  • Advocate for clients’ welfare and rights.
  • Work collaboratively with nurses, paramedics, caregivers, pharmacists, and other professionals.
  • Maintain accurate and confidential client records.
  • Conduct home visits where appropriate.
  • Participate in community outreach programmes.
  • Provide psychosocial support to patients and families dealing with illness or disability.
  • Identify safeguarding concerns and follow appropriate professional referral procedures.
